Plumbing Service Agreement
Any work requested not stated in this agreement will be performed at the current hourly pricing. Any work performed other than repairs made will have no warranty written or expressed. Due to age, condition, and lack of previous maintenance some valves may need to be replaced due to leakage that may occur as a result of testing, customer is aware that any valves needing replacement will be replaced only at the customer's request and at the customer's expense.
Mountain City Service will not perform any repair, other than washer replacement, without permission from the customer. The customer understands that priority service refers to expedited scheduling and does not imply an immediate response. Agreement to automatically terminate one year from the signature date. All credit card transactions will have an additional 3%.
A Service Agreement Customer Receives
- Annual check of the plumbing system, including minor faucet repairs (washers only)
- 15% discount on labor and materials, should additional service work be required during the year
- Priority emergency service
- Peace of mind knowing your system is operating safely and efficiently
- Annual backflow testing for irrigation or domestic water is an additional $150

The pricing for this annual service will be at a cost of $300.00 to be paid on an annual basis.
- Check/test all toilets for operation and leakage
- Check all faucets for leaks
- Check all exposed hot and cold water supply lines
- Check all sink piping
- Check and clean all fixture drains
- Check washing machine hoses
- Check all lawn faucets for leaks
- Water heater model
- Serial #
- Check all water shut off valves
- Treat all drains with cleaner/treatment
- Check water system for leaks and notify the customer (check at the city meter)
- Flush water heater
- Test water heater elements for efficient performance
